Use
Once an agreement or court order has determined how the Member's pension will be divided, the
Request for Transfer from Unmatured Defined Contribution Plan (Form 3) - BC
is used to transfer out the spouse's share of a Member's unmatured Defined Contribution plan.
The Member's spouse uses this form after a marriage breakdown in British Columbia.

Information Needed to Complete the Form
To complete the form, the spouse must include:
- Plan information,
- spouse's personal information,
- Plan Member personal information, and
- information related to the spouse's request.

|
Notes
A copy of the agreement or court order on which the entitlement date and
division of pension is based must be submitted with this form.
The spouse should have already submitted, or be submitting with this form, a completed
Claim of Spouse to Interest in Member's Pension (Form 1) - BC
If the agreement or court order between the spouse and Member was made prior to July 1, 1995,
the spouse should complete the
Request in Relation to a Matured Pension Divided under an Agreement or Court Order made before July 1, 1995 (Form 5) - BC
instead of this form.
Questions on completing the form should be directed to the Plan
Administrator.
Completed forms should be forwarded to the Plan
Administrator.
