Use
The Member's former spouse uses this form after a marriage breakdown (divorce) in British Columbia.
If the agreement or court order dividing the Member's pension was made before July 1, 1995, the spouse should submit the
Request in Relation to a Matured Pension Divided under an Agreement or Court Order made before July 1, 1995 (Form 5) - BC
to become a 'Limited Member' of the Pension Plan and to direct the Plan to split the Member's pension at retirement. |

Information Needed to Complete the Form
To complete the form, the spouse must include:
- Plan information,
- spouse's personal information,
- Member personal information, and
- information related to the spouse's claim.

Notes
If the agreement or court order was made after July 1, 1995, the spouse should complete a
Claim of Spouse to Interest in Member's Pension (Form 1) - BC
, and a
Request for Transfer from Unmatured Defined Contribution Plan (Form 3) - BC
instead of this form.
A copy of the agreement or court order on which the division of the pension is based must be submitted with this form.
Questions on completing the form should be directed to the Plan
Administrator.
Completed forms should be forwarded to the Plan
